Class Terms & Agreement – This is the same Agreement that I use in all of my classes! It is in Word format so that you can adapt it to your particular needs but gives you a nice framework from which to start. It includes class rules & guidelines, refund & cancellation policies and a confidentiality release.
Class and End of Series Evaluation Forms – This two page PDF contains two different type of evaluations. The first page is meant to be printed, cut into half sheets and used as a quick evaluation at the end of each class. The second page is a full-sheet evaluation to be used at the end of the entire class series. There are many things that are better done by email but evaluations are not one of them! You will get more filled out and better feedback if it’s done in person and the information is fresh!
Class Sign-in Sheet – A great form to keep in your class file. There are spaces for each student’s name, cell phone number (with permission to text), and email (with permission to add to newsletter). Getting this information on the first day allows you to put together a group text list. That way if you have an emergency, if there is inclement weather, or if you need to pass on last minute information, you will have a quick method of communication set up and ready to go!
Class Checklist – Avoid the last minute panic! Go through this list for each class series to make sure you haven’t missed anything. It includes finding a location, method of registration, confirmation and reminder emails, supplies, snacks and more. Each item on the list also has tips & ideas!
